Refractive Lens Exchange in Medellin: When Replacing the Lens Beats LASIK

After about 45, the part of your eye that needs fixing may not be the cornea at all.

The short versionRLE replaces your natural lens with an artificial one, the same operation as cataract surgery done before a cataract forms. It can treat distance vision and, with multifocal or extended-range lenses, some near vision. Why age changes the question

In your twenties and thirties, the refractive conversation is about the cornea. From the mid-forties, presbyopia arrives for everyone: the natural lens stiffens and close focus fades. LASIK can correct distance vision perfectly and you will still need reading glasses. Surgeons then offer different tools: monovision within laser surgery (see monovision LASIK) or refractive lens exchange.

RLE removes the clear natural lens through a small incision and implants an intraocular lens (IOL). Because the natural lens is gone, you will never develop a cataract in that eye. That is the long-term trade that makes RLE attractive at lens-change ages.

What it costs in Colombia

A Colombian ophthalmology referral site puts lens implant surgery at about COP 5 million per eye for monofocal lenses up to more than COP 8.5 million per eye for multifocal lenses, with an overall range of COP 5 to 10.5 million per eye. Premium lenses and well-known clinics sit at the top of that range.

RLE vs monovision LASIK for moderate to high myopes

One large comparison summarized in Review of Ophthalmology (590 RLE patients and 609 monovision LASIK patients) found that among moderate to high myopes, more than 94 percent favored monovision LASIK against 79.1 percent for RLE, with no significant satisfaction difference in the other refractive groups. That does not make LASIK the answer for everyone over 45. It does mean a myope with healthy corneas and no early cataract should hear both options before choosing the bigger surgery.

Risks worth naming

RLE is intraocular surgery. Besides the night-vision effects some lenses cause, the risk list includes infection, inflammation, raised eye pressure, and retinal detachment, a risk that is more discussed in highly myopic eyes. A later haze behind the implant (posterior capsule opacification) is common and usually treated with a quick laser in the office. None of this is a reason to panic. It is a reason to choose an experienced cataract and refractive surgeon, and to plan follow-up at home.

Planning the trip

Most surgeons do one eye, then the second a short interval later, so the trip is longer than LASIK. Ask for the full schedule, which checks are required before you fly, and who at home can do later checks.
