What we collect
If you use the quote request, we may collect contact details, country, procedure interest, current vision-correction method, planning/timing information, self-reported answers about prior evaluation or prescription change, priorities, and free-text notes. We also record technical and attribution data such as the page that led to the form, referral source and UTM parameters.
What not to submit
Do not use the initial website form to send scans, medical records, government identification, insurance numbers, payment-card data, passwords, or other highly sensitive records. A participating clinic can tell you how to transmit clinical records securely if needed.
Provider sharing
If you submit a patient quote request and check the required consent box, MedellínLASIK may share your request and contact information with up to three ophthalmology providers that may be able to assist you. Participating providers may compensate MedellínLASIK for introductions or resulting business. This does not make MedellínLASIK a medical provider and does not determine which treatment is appropriate.
Service providers and analytics
The site may use web analytics and a Google Apps Script-based lead routing workflow to process submissions. Information may therefore be processed by service providers used to operate the site and lead-management workflow.
Retention and requests
We retain inquiry information as reasonably necessary to operate the matching service, prevent abuse, measure lead quality and maintain business records. To request access, correction or deletion of information you submitted, contact info@medellinlasik.co. Legal or operational obligations may require retention of some records.
Third-party providers
Once information is shared with a participating provider, that provider's own privacy practices apply to its subsequent collection and handling of medical information.
